Participant-specific support for complex disability-related health needs, supported by appropriate clinical assessment, care planning, training and nursing oversight.
High Intensity Supports
Open Sky supports participants with a range of complex disability-related health support needs.
Where required, supports are delivered in accordance with participant-specific assessments, care plans, clinical guidance, worker training and competency requirements.
Depending on the participant’s assessed needs, this may include:
- Complex bowel care
- Enteral feeding, including PEG support
- Urinary catheter care
- Diabetes-related support
- Subcutaneous injections
- Other participant-specific high intensity supports within our approved scope of service
COMMUNITY NURSING
Our Registered Nurses provide participant-specific nursing support based on assessed clinical needs and the participant’s NDIS plan.
Depending on individual requirements, nursing support may include:
- Nursing assessments and care planning
- Medication assessment and management
- Wound assessment and wound care
- Continence assessment and support
- Catheter assessment and management
- Clinical monitoring
- Falls and health risk assessment
- Participant-specific clinical education
- Clinical guidance and competency support for disability support workers where appropriate
